Q. I am a physician/advanced practice clinician nurse/therapist working with patients with diabetes. Can I participate in this activity?
A. Yes! This activity is designed to address delivery of care by individual clinicians. While CME credit will be awarded to physicians, all other clinicians completing this activity will receive a certificate of participation, reflecting that the activity was certified for AMA PRA credit.

Q. Do I have to complete all modules to receive credit?
A. Yes. To receive credit, physicians must complete the overall pre-assessment, and complete the post-assessments for each of the four modules. You may participate in the supplemental education as directed and return to the Diabetes Formative Assessment where physicians can then be awarded credit for completion of the activity.

Q. May I pause during the activity in the middle of a pre- or post-assessment and return at a later time to complete it?
A. If you need to leave the activity at any time, your progress to that point and your place within the activity will be saved. To return to the activity, you will need to log in with your user name and password.

Q. Can I receive credit for the supplemental education?
A. If the supplemental education is certified for credit, you may also receive credit from another accredited provider for completing that educational activity.

Completing the Diabetes Formative Assessment

  1. Take the pre-assessment. There are a total of 39 questions in the formative assessment. Learners have the option of stopping and resuming the pre-assessment at a later time. However, the pre-assessment must be completed in its entirety before proceeding to any other portion of the activity.

    Additionally, the questions contained within the pre-assessment are also grouped according to topics which allows for identification of areas of excellence (strengths) or areas that may need further study and improvement (weaknesses). Following the pre-assessment, all users will receive a (n):

    • average score (based on the 39 questions) determined as follows

      • mastery, ≥89%

      • proficient, ≥80% and ≤88%

      • satisfactory, ≥70% and ≤79%

      • needs improvement, ≤69%

    • module-based score (based on 8-12 questions per module)

    • topic-based score

    • percentile ranking, compared to peers with similar educational background (ie, physician, nurse, advanced practice clinician (PA, NP, etc))

  2. Complete the educational module. Each module consists of supplemental education provided via external links. Each link will direct you to an external site where you can follow the requirements for the program and may earn additional CME credit. A learner may choose the order in which to view each of the links, and may complete as many or few supplemental educational activities as needed to achieve desired score in the post-assessment.

  3. Complete the post-assessment. The post-assessment will only contain questions related to that specific module (a maximum of 12 questions per module). To receive credit you must complete the post assessment for each of the four modules, and achieve greater than 70% (satisfactory).

    Following the module-based post-assessment, learners will be presented with:

    • their answer, the correct answer (if different), rationale*, and source

    • an average score determined as follows

      • mastery, ≥89%

      • proficient, ≥80% and ≤88%

      • satisfactory, ≥70% and ≤79%

      • needs improvement, ≤69%

    • a topic-based score

    • a percentile ranking, compared to peers with similar educational background (ie, physician, nurse, advanced practice clinician (PA, NP, etc))

    • their pre-assessment score

    * Please note that at times external web addresses are supplied as part of the rationales in the post-assessment. Learners are welcome to access these websites to further their education but are hereby notified that any external content is independent of the Diabetes Formative Assessment.

  4. Complete the evaluation. The evaluation will help the activity planners to assess the utility of the exercise and provide constructive feedback to improve future activities.

  5. Receive credit for the activity. Additionally, learners have the option to download a certificate of participation.

Remember that at any point in the activity, learners may close the program and log back in at a later time to resume their participation. Reminders will be auto-emailed to participants following one week of inactivity (and an incomplete activity status in their dashboard).
